Bay State Baseball Tournament of Champions Injury Waiver Form Requirements Bay State Baseball Tournament of Champions (BSBTC) requires the manager/coach of each team to complete and return to Bay State Baseball Tournament of Champions the Injury Waiver Form as a prerequisite to participation in the Bay State Tournament of Champions. The permission to participate granted by parent[s]/guardian[s] shall extend from year to year until such time as the player graduates from BSBTC or otherwise ceases participation in the program. Please complete the Injury Waiver Form and return to: Brian Doherty or bring to the Coaches meeting on June 27th. To print a form for your team please select Team Injury Waiver Form
